Everything you type into the GP Appointment Summary Builder stays on your own device. It is never sent to us or to anyone else.
Who we are
Heard and Charted is a small UK shop that makes printable symptom trackers and GP appointment planners. It's run by Gary Cope. You can contact us through our Etsy shop.
What happens to what you type
- The tool runs entirely in your web browser. Your answers are never uploaded, and we can't see them.
- Your browser saves your answers on your device (in its local storage), so they're still there if you close the page and come back.
- To remove them, press Clear everything, or clear your browser's site data. On a shared or work computer, clear them when you've finished.
- Printing and "Copy as text" happen on your device too. Where the copy goes after that is up to you.
Cookies and tracking
We don't use cookies, advertising trackers or analytics on this site. The fonts are stored on this site, so your browser doesn't contact Google or any other font service.
Our web host
This site is hosted by Cloudflare. Like any web host, Cloudflare receives basic technical information when your browser asks for a page, such as your IP address and browser type, so it can deliver the page and protect the site from attacks. This never includes what you type into the tool. You can read Cloudflare's privacy policy.
Links to our shop
Links to our planners go to Etsy. If you follow one, Etsy's own privacy policy applies there.
Not medical advice
The tool helps you organise what you want to say. It doesn't diagnose anything or look at your answers. Always talk to your GP or a qualified clinician about your symptoms. If you need help urgently, call 999, or NHS 111 for advice.
Changes
If we change how the site works, for example by adding an email sign-up, we'll update this page and the date at the top first.
